If you have waited 4-6 weeks after filling out an application for a videophone and have not yet been contacted by Sorenson, please submit a status request form. A representative from Sorenson will contact you soon.

Please complete this form and click “Submit” when finished. List the same name and email address you listed on your application for a Sorenson videophone. This will help us find your application more quickly. Once you’ve submitted this form, a customer service representative will contact you via email.
Reasons why you may not have received a Sorenson videophone:
- Your application was incomplete or inaccurate.
- You haven't replied to a follow-up email from an SVRS representative.
- Either your ISP doesn't support a 256 Kbps connection for the Sorenson videophone or you don't already have a high-speed Internet connection installed.
- There's not an SVRS installer in your area.
